Atomic Absorption Spectroscopy: Interference01:25

Atomic Absorption Spectroscopy: Interference

2.3K
Interference leads to systematic error in atomic absorption (AA) measurements by enhancing or diminishing the analytical signal or the background. These interferences can be grouped into three main categories: spectral interference, chemical interference, and physical interference.
Spectral interference occurs when signals from other elements or molecules overlap with the analyte signal, falsely elevating or masking the analyte's absorbance. This interference can be corrected using Zeeman,...

Inductively Coupled Plasma-Mass Spectrometry (ICP-MS): Interferences

1.7K
Inductively coupled plasma–mass spectrometry (ICP–MS) is a highly selective and sensitive technique for accurate elemental analysis. Though the analysis of ICP–MS mass spectra is comparatively straightforward, it is affected by spectroscopic and non-spectroscopic interferences. Spectroscopic interferences arise when the plasma contains ionic species with an m/z value the same as the analyte ion. Spectroscopic interference can be categorized as isobaric, polyatomic ions, and...

Pharmaceutical Alternatives: Stability-Related Therapeutic Nonequivalence

Generic intravenous (IV) drugs are considered bioequivalent to their branded counterparts due to their 100% bioavailability upon administration. However, variations in stability among different drug products can significantly influence their therapeutic performance, even if they are pharmaceutically equivalent.Cefuroxime, a prophylactic antimicrobial, is often used as a single-dose IV injection for patients undergoing coronary artery bypass grafting surgery. A 3 g dose typically provides...

Testosterone: Functions and Regulation

3.5K
The intricate hormonal interplay essential for male reproductive health begins with the release of gonadotropin-releasing hormone (GnRH) by the hypothalamus. This hormone prompts the pituitary gland to secrete follicle-stimulating hormone (FSH) and luteinizing hormone (LH). LH targets the Leydig cells in the testes, stimulating them to produce and release testosterone. In concert with testosterone, FSH acts on the Sertoli cells within the seminiferous tubules to facilitate the release of...

Therapeutic Drug Monitoring: Drug Analysis Methods01:26

Therapeutic Drug Monitoring: Drug Analysis Methods

346
Therapeutic Drug Monitoring (TDM) is a clinical practice that measures specific drug levels in a patient's blood or body tissues to tailor drug therapy effectively. This monitoring is critical for managing drugs with narrow therapeutic indices like digoxin and phenytoin, ensuring they are both safe and effective. For instance, monitoring theophylline levels in asthma patients involves precision and sensitivity to adjust doses according to individual responses to therapy, ensuring efficacy and...

Therapeutic Drug Monitoring: Affecting Factors

438
Therapeutic Drug Monitoring (TDM) is the clinical practice of measuring specific drug levels in a patient's blood or body tissues to manage and optimize therapy. TDM is crucial for drugs with narrow therapeutic windows, like warfarin and phenytoin, where incorrect doses can lead to treatment failure or severe side effects. This monitoring ensures the dosage administered is within a safe and effective range. The factors affecting therapeutic drug monitoring include:Patient-Specific Factors:a.

Summary

Norethisterone medication can cause falsely high testosterone levels in immunoassay tests. Liquid chromatography-tandem mass spectrometry accurately measures testosterone, unaffected by this drug interference.

Area of Science:

  • Clinical Chemistry
  • Endocrinology
  • Pharmaceutical Analysis

Background:

  • Identified artifactually elevated serum testosterone concentrations in three patients due to norethisterone medication.
  • Investigated drug interference using an external quality assessment scheme for testosterone measurement.

Purpose of the Study:

  • To assess the interference of norethisterone on testosterone measurements across various analytical methods.
  • To quantify the degree of interference in commercially available immunoassays and liquid chromatography-tandem mass spectrometry (LC-MS/MS).

Main Methods:

  • Distributed serum samples with varying norethisterone concentrations through the UK external quality assessment scheme (UK NEQAS).
  • Evaluated interference in different immunoassay and LC-MS/MS testosterone measurement methods.

Main Results:

  • The Roche E170 Modular immunoassay method showed significant interference, yielding apparent testosterone levels over 5 nmol/L in patients taking norethisterone.
  • Interference was reproducible by adding norethisterone to serum samples, confirming the drug's impact.

Conclusions:

  • The Roche system exhibited the most substantial interference, while the Siemens ADVIA Centaur assay showed a minor effect.
  • Other immunoassays may be affected to a lesser extent by norethisterone.
  • LC-MS/MS methods for testosterone measurement were not affected by norethisterone interference.
