Background:

  • Post-traumatic nasolacrimal drainage obstruction is a rare complication of naso-orbito-ethmoid fractures.
  • Dacryocystorhinostomy (DCR) is the standard surgical treatment for such obstructions.

Observation:

  • A patient presented with recurrent lacrimal sac abscess after a naso-orbito-ethmoid fracture.
  • The patient had a history of multiple incision and drainage procedures and two failed DCR surgeries.
  • Extensive nasal synechiae complicated previous surgical attempts.

Findings:

  • Dacryocystectomy was performed due to the complexity and previous treatment failures.
  • The patient experienced significant improvement and remained symptom-free for 14 months post-surgery.
  • This represents the first reported case of successful dacryocystectomy for post-traumatic dacryocystitis.

Implications:

  • Dacryocystectomy may be a viable alternative treatment for complex, recurrent post-traumatic nasolacrimal drainage obstruction.
  • This case expands the surgical options for managing challenging lacrimal drainage issues following facial trauma.
