Adrenergic Antagonists: Pharmacological Actions of β-Receptor Blockers01:27

Adrenergic Antagonists: Pharmacological Actions of β-Receptor Blockers

1.8K
β-receptor blockers significantly impact the cardiovascular system by counteracting catecholamine-induced sympathetic responses. These medications decrease heart rate, contractility, and cardiac output, potentially leading to cardiac depression, life-threatening bradycardia, and death. Therapeutically, β-blockers function as mild antihypertensives and are utilized in treating angina pectoris and cardiac arrhythmias. However, nonselective β-blockers inhibit β2-receptors in...

Esophageal Varices-II: Clinical Features and Management

893
Esophageal varices often manifest as gastrointestinal bleeding episodes, presenting symptoms like hematemesis (vomiting of blood), hematochezia (passing fresh blood via the rectum), and melena (black, tarry stools). Other signs can include weight loss, anorexia, abdominal discomfort, jaundice, pruritus, altered mental status, and muscle cramps.
In the initial assessment, a thorough review of the patient's medical history is vital to identify risk factors such as liver disease, alcohol...

Open Angle Glaucoma: Treatment01:27

Open Angle Glaucoma: Treatment

1.1K
In open-angle glaucoma, the iridocorneal angle remains open, but the trabecular meshwork becomes stiff, slowing down the outflow of aqueous humor. This causes a buildup of aqueous humor in the anterior chamber, leading to a sudden increase in intraocular pressure. The treatment for open-angle glaucoma focuses on reducing the elevated intraocular pressure by either decreasing the secretion of aqueous humor or increasing its outflow.
Drugs such as carbonic anhydrase inhibitors, α2- and...

Adrenergic Antagonists: ɑ and β-Receptor Blockers

1.4K
Third-generation β-blockers, such as labetalol and carvedilol, represent a significant advancement in managing cardiovascular conditions. Unlike conventional β-blockers, which can induce peripheral vasoconstriction, third-generation drugs block α1 adrenoceptors. This promotes vasodilation through several mechanisms, such as increased nitric oxide production, inhibition of calcium ion entry, opening of potassium ion channels, and antioxidant action. Labetalol, for instance, is...

Aneurysm III: Interprofessional Care

478
Aneurysm management involves either conservative medical therapy or surgical intervention, depending on the size and symptoms of the aneurysm. Conservative management is generally reserved for smaller, asymptomatic aneurysms, while larger or symptomatic aneurysms often necessitate surgical repair.Conservative Medical TherapyFor small, asymptomatic aneurysms, particularly abdominal aortic aneurysms (AAA) less than 5.5 centimeters in diameter, conservative medical therapy is recommended. This...

Angle Closure Glaucoma: Treatment01:28

Angle Closure Glaucoma: Treatment

1.7K
Angle-closure glaucoma, or closed-angle glaucoma, is an eye condition where the iris bulges out and blocks the iridocorneal angle, resulting in a buildup of aqueous humor and increased intraocular pressure. Immediate medical attention is necessary due to the sudden onset of symptoms. The treatment for angle-closure glaucoma includes short-term and long-term approaches. Short-term treatment involves using eye drops like pilocarpine to lower intraocular pressure by increasing aqueous humor...

Summary

Most infantile hemangiomas resolve spontaneously. However, a large orbital hemangioma obscuring the eye in a 5-month-old infant was successfully treated with oral propranolol, preserving vision.

Area of Science:

  • Ophthalmology
  • Pediatrics
  • Dermatology

Background:

  • Infantile hemangiomas are common benign vascular tumors in infants.
  • While many resolve spontaneously, some require intervention due to functional impairment or complications.
  • Orbital hemangiomas can threaten vision and require prompt management.

Observation:

  • A 5-month-old infant presented with a large left-sided orbital hemangioma.
  • The hemangioma completely obscured the palpebral fissure, posing a risk to vision.
  • The infant had no other significant medical history.

Findings:

  • The infant was successfully managed with oral propranolol therapy.
  • Propranolol led to significant regression of the orbital hemangioma.
  • Visual function was preserved, and the palpebral fissure was no longer obscured.

Implications:

  • Oral propranolol is an effective treatment for large orbital infantile hemangiomas.
  • Early intervention with propranolol can prevent long-term visual impairment.
  • This case highlights the importance of considering systemic propranolol for visually significant infantile hemangiomas.
