Background:

  • The necessity of radiotherapy for all early breast cancer patients warrants investigation.

Purpose of the Study:

  • To evaluate the role of adjuvant radiotherapy in early breast cancer.
  • To determine if a low-risk subgroup exists for whom radiotherapy omission is feasible.
  • To analyze local recurrence rates in patients who did not receive radiotherapy.

Main Methods:

  • A comprehensive literature search was performed using Medline and Ovid databases (1965-2010).
  • Search terms included 'role of radiotherapy', 'early breast cancer', and 'omission of radiotherapy'.
  • Included were 14 randomized controlled trials published in English involving adult human subjects.

Main Results:

  • Local recurrence rates varied significantly (0.8%–35%) in patients who omitted radiotherapy.
  • Identified low-risk characteristics include older age, smaller tumor size, absence of lymphovascular invasion, and low-to-moderate tumor grade.
  • No clearly defined low-risk group for whom radiotherapy omission is definitively safe was established.

Conclusions:

  • While certain patient and tumor characteristics are associated with lower risk, they do not currently permit the omission of radiotherapy.
  • Further research is needed to precisely define low-risk populations for potential radiotherapy de-escalation in early breast cancer.
  • Adjuvant radiotherapy remains a critical component in reducing local recurrence for most early breast cancer patients treated with breast conservation.
