Retroperitoneal laparoscopic partial nephrectomy for moderately complex renal hilar tumors
1Department of Urology, Peking University First Hospital and Institute of Urology, Peking University, National Urological Cancer Center, Beijing, China.
Retroperitoneal laparoscopic partial nephrectomy (LPN) is a feasible and safe option for complex renal hilar tumors. This minimally invasive technique shows promising outcomes with no recurrence in initial patient cases.
Area of Science:
- Urology
- Minimally Invasive Surgery
- Oncology
Background:
- Renal hilar tumors present surgical challenges.
- Partial nephrectomy is preferred for preserving renal function.
- Retroperitoneal laparoscopic partial nephrectomy (LPN) offers a minimally invasive alternative.
Purpose of the Study:
- To evaluate the initial experience with retroperitoneal LPN for moderately complex renal hilar tumors.
- To assess the feasibility and safety of the retroperitoneal laparoscopic approach for these specific tumors.
Main Methods:
- Retrospective analysis of 14 patients undergoing retroperitoneal LPN for renal hilar tumors.
- Tumors evaluated using the R.E.N.A.L. nephrometry scoring system.
- A three-port retroperitoneal approach with dorsal incision was utilized.
Main Results:
- All 14 procedures were completed successfully without conversion to open surgery.
- Low complication rate (2 cases of perirenal fluid collection) managed conservatively.
- Favorable perioperative outcomes including mean operative time (134.3 min), blood loss (133.2 ml), warm ischemia time (30.6 min), and hospital stay (4.7 days).
- No recurrence or metastasis observed at a mean follow-up of 18.6 months.
Conclusions:
- Retroperitoneal LPN is a feasible, safe, and effective treatment for moderately complex renal hilar tumors.
- The approach is technically demanding but can be recommended for complex cases in experienced surgical hands.
