Narcolepsy is a chronic sleep disorder characterized by pervasive, uncontrolled sleepiness and other sleep disturbances. One of its hallmark symptoms is an abrupt transition to REM sleep upon falling asleep, which causes symptoms typically associated with this phase to occur unexpectedly during wakefulness. These include the following symptoms, which typically last from a minute or two to half an hour.

Sleep is an essential physiological process vital to maintaining overall well-being. The reticular activating system (RAS), a network of neurons in the brainstem, regulates wakefulness and sleep. While it may seem passive, sleep consists of distinct cycles, each with its unique characteristics and functions. Two key sleep phases are non-rapid eye movement (NREM) and  rapid eye movement (REM).

Narcolepsy with cataplexy shows brain microstructural changes in the frontal gyrus and amygdala, unlike narcolepsy without cataplexy. Diffusion tensor imaging (DTI) reveals these differences, suggesting distinct pathological mechanisms.

Background:

  • Narcolepsy is a sleep disorder characterized by excessive daytime sleepiness.
  • Narcolepsy can occur with or without cataplexy, a sudden loss of muscle tone.
  • Diffusion tensor imaging (DTI) using magnetic resonance imaging (MRI) can detect microstructural changes in the brain by analyzing water molecule diffusivity.

Purpose of the Study:

  • To evaluate brain microstructural abnormalities in narcolepsy using DTI.
  • To investigate differences in brain microstructure between narcolepsy with cataplexy (NA/CA) and narcolepsy without cataplexy (NA w/o CA).
  • To compare brain microstructure in narcolepsy patients with healthy controls (NC).

Main Methods:

  • Enrolled 12 drug-naive NA/CA patients, 12 drug-naive NA w/o CA patients, and 12 age-matched NC.
  • Acquired fractional anisotropy (FA) and apparent diffusion coefficient (ADC) maps using DTI-MRI.
  • Statistically compared FA and ADC maps across the three groups using voxel-based one-way ANOVA.

Main Results:

  • The NA/CA group exhibited higher ADC in the left inferior frontal gyrus and amygdala, and lower ADC in the left postcentral gyrus compared to NC.
  • NA/CA showed higher ADC in the right inferior frontal gyrus and higher FA in the right precuneus compared to NA w/o CA.
  • No significant FA or ADC differences were found between NA w/o CA and NC groups.

Conclusions:

  • Microstructural changes in the inferior frontal gyrus and amygdala are present in NA/CA but not in NA w/o CA.
  • These findings suggest distinct pathological mechanisms for narcolepsy with and without cataplexy.
  • Narcolepsy without cataplexy may represent a broader condition with no significant brain imaging differences from healthy controls.
