Purpose of the Study:

  • To investigate the clinical presentation and surgical management of fourth branchial anomalies.
  • To analyze the anatomical variations and treatment outcomes in a series of patients.

  • Retrospective review of twelve cases of fourth branchial anomalies treated between January 2005 and April 2012.
  • Surgical procedures included dissection of the recurrent laryngeal nerve, partial thyroidectomy, and selective neck dissection (levels II, III, IV, VI).

  • Anomalies frequently coursed posterior to the thyroid gland and into the pyriform sinus.
  • Recurrent laryngeal nerve injury occurred in one case, resulting in vocal cord paralysis.
  • No recurrence was observed in 10 out of 11 cases with follow-up, with a median follow-up of 24 months.

Conclusions:

  • Congenital fourth branchial fistulas exhibit significant variability in presentation.
  • Protecting the recurrent laryngeal nerve is paramount; partial thyroidectomy may be beneficial.
