Background:

  • Basal cell carcinoma (BCC) is the most prevalent human malignancy, characterized by invasive growth and local tissue destruction.
  • While most BCCs have high cure rates (>90%), a rare subset, metastasizing BCC (MBCC), presents significant morbidity and mortality risks.

Observation:

  • A case report details a 66-year-old male with a large, ulcerative BCC on his flank.
  • Clinical and imaging revealed extensive local invasion and distant metastases to lymph nodes, lungs, liver, bone marrow, and bone.

Findings:

  • The patient succumbed to the disease 7 months post-diagnosis.
  • Identifying potentially metastasizing BCCs pre-emptively remains a clinical challenge.

Implications:

  • Early and adequate treatment for all BCC diagnoses is crucial due to the unpredictable metastatic potential.
  • This case highlights the aggressive nature and poor prognosis of MBCC, emphasizing the importance of vigilant patient monitoring and timely therapeutic interventions.
