Summary

A rare retroperitoneal lipoma caused nutcracker-like phenomenon in a 65-year-old man. Surgical resection of the lipoma resolved his flank pain and hematuria, with no recurrence for 10 years.

Area of Science:

  • Urology
  • Nephrology
  • Surgical Oncology

Background:

  • Retroperitoneal lipomas are rare tumors.
  • Nutcracker-like phenomenon, or renal vein compression, is an uncommon complication of retroperitoneal masses.
  • This case highlights an unusual presentation of a common benign tumor.

Purpose of the Study:

  • To report a rare case of a retroperitoneal lipoma causing nutcracker-like phenomenon.
  • To describe the diagnostic findings and successful surgical management.
  • To emphasize the long-term outcome of surgical intervention.

Main Methods:

  • A 65-year-old male patient presented with chronic left flank pain and macrohematuria.
  • Computed tomography (CT) imaging identified a 30 mm retroperitoneal lipoma compressing the left renal vasculature.
  • The patient underwent successful retroperitoneoscopic resection of the lipoma.

Main Results:

  • The lipoma caused significant compression of the left renal artery and vein, mimicking the nutcracker syndrome.
  • Post-operative recovery was uneventful.
  • The patient remained asymptomatic with no signs of recurrence at a 10-year follow-up.

Conclusions:

  • Retroperitoneal lipomas can present with rare complications like the nutcracker-like phenomenon.
  • Retroperitoneoscopic resection is a safe and effective treatment for symptomatic retroperitoneal lipomas.
  • Complete resolution of symptoms and long-term disease-free survival can be achieved.
