Background:

Complex hand trauma requires specialized surgical techniques for optimal functional recovery.
Thumb amputation at the metacarpal level presents significant challenges due to associated soft tissue and neurovascular damage.

Observation:

A patient presented with incomplete left thumb amputation at the metacarpal level.
Associated injuries included extensor pollicis longus (EPL) and flexor pollicis longus (FPL) tendon avulsion from the tenomuscular junction.

Findings:

Metacarpal bone fixation was performed, followed by ulnar digital artery anastomosis and digital nerve coaptation.
Reconstructive procedures included transfer of the extensor indicis proprius (EIP) to the EPL tendon and side-to-side suturing of the FPL to the index finger's deep flexor tendon.
