    Summary

    Steroid use significantly contributes to peptic ulcer perforations, particularly in patients over 50. A sudden increase in steroid dosage (pulse) often precedes these life-threatening events.

    Background:

    • Peptic ulcer perforation is a serious complication.

    Purpose of the Study:

    • To determine the incidence and outcomes of peptic ulcer perforations associated with steroid treatment.
    • To identify risk factors and patterns preceding steroid-associated perforations.

    • Retrospective chart review of 151 peptic ulcer perforations from 1974-1984.
    • Analysis of patient demographics, steroid use, concurrent conditions, treatment, and mortality.

    Main Results:

    • 25% of perforations were linked to steroid use.
    • Mortality was 85% in patients over 50 vs. 17% in younger patients.
    • A steroid pulse preceded perforation in 21 patients.

    Conclusions:

    • Steroid-associated peptic ulcer perforations are a significant cause of mortality, especially in older adults.
    • Steroid pulse therapy may increase the risk of perforation.
    • Early recognition and management are crucial for improving outcomes.
