    Summary

    Alzheimer's disease neurofibrillary degeneration in the hypothalamus primarily affects neurons projecting to affected cortical areas. This supports the neuron-to-neuron transmission hypothesis of Alzheimer's disease progression.

    Background:

    • Alzheimer's disease (AD) is characterized by neurofibrillary degeneration.
    • The hypothalamus, a key brain region, has not been fully characterized in AD neuropathology.

    Purpose of the Study:

    • To investigate neurofibrillary degeneration in the hypothalamus in Alzheimer's disease.
    • To identify specific neuronal populations affected by neurodegeneration in the hypothalamus.

    Main Methods:

    • Combined acetylcholinesterase (AChE) and thioflavin-S staining on post-mortem brain tissue.
    • Examination of hypothalamic regions in 3 Alzheimer's disease cases and 3 control brains.

    Main Results:

    • Neurofibrillary degeneration was localized to three specific AChE-positive cell populations within the hypothalamus: lateral tuberal, lateral posterior, and tuberomammillary nuclei.
    • These affected neuronal populations are known to project to the cerebral cortex in animal models (rat and monkey).

    Conclusions:

    • Hypothalamic neurofibrillary degeneration in Alzheimer's disease predominantly involves neurons innervating cortical areas affected by the disease.
    • Findings support the hypothesis that Alzheimer's disease pathology may spread trans-neuronally through established neural pathways.
