Summary

Cardiac resynchronization therapy (CRT) shows nonsignificant overall effects on cognitive impairment (CI) in heart failure patients. However, CRT may significantly improve executive functioning and attention, though more research is needed due to study limitations.

Background:

  • Cognitive impairment (CI) is prevalent in congestive heart failure (CHF) patients, affecting memory, attention, problem-solving, and motor speed.
  • Few studies have explored the impact of cardiac resynchronization therapy (CRT) on cognitive performance in CHF patients.

Purpose of the Study:

  • To systematically review and evaluate published studies on the effects of CRT on neuropsychological function in patients with CHF.

Main Methods:

  • A systematic literature search was conducted using terms: CRT, cognition, cognitive, and neurocognitive.
  • Data extraction and computation of effect sizes (Hedges' g) were performed.
  • A fixed-effect model was used to estimate summary effects for domain-specific and global cognitive scores.

Main Results:

  • Only three studies met the inclusion criteria, exhibiting discordant results and methodological limitations.
  • The overall meta-analysis of cognitive outcomes was nonsignificant (Hedges' g = 0.131).
  • Significant pooled effects were observed for executive functioning (Hedges' g = 0.374) and attention (Hedges' g = 0.343).

Conclusions:

  • While CRT may offer domain-specific cognitive benefits (executive function, attention) in CHF patients, the overall impact is not statistically significant.
  • Methodological limitations in the included studies restrict the validity and clinical relevance of these findings.
  • Further high-quality research is necessary to definitively assess CRT's role in improving cognitive function in CHF.
