French hypothermia treatment (HT) for newborns with hypoxic-ischemic encephalopathy (HIE) shows significant deviations from guidelines. Many cases experience delayed treatment or lack of adherence to indications, highlighting challenges in optimal perinatal care.
Area of Science:
- Neonatal medicine
- Perinatal care
- Neurology
Background:
- Hypoxic-ischemic encephalopathy (HIE) is a serious condition in term newborns.
- Hypothermia treatment (HT) is a standard therapy for HIE.
- Adherence to established guidelines is crucial for optimal patient outcomes.
Purpose of the Study:
- To describe the current practice of HT for HIE in France.
- To analyze deviations from French Society of Neonatology guidelines for HT.
- To identify areas for improvement in perinatal care for HIE.
Main Methods:
- A national database recorded HIE cases treated with HT from May 2010 to March 2012.
- Newborns were categorized into optimal HT (OHT), late HT (LHT), and non-indicated HT (NIHT) groups based on guidelines.
- Data on treatment timing, clinical indicators, and outcomes were collected.
Main Results:
- 35% of HIE cases treated with HT deviated from French guidelines, with 22% due to delayed HT initiation and 13% due to lack of clinical indication.
- Newborns in the LHT group received HT at a mean of 12 hours of life.
- Complication rates were higher in the OHT group (52%) compared to LHT (38%) and NIHT (29%), despite similar death/neurological abnormality rates at discharge.
Conclusions:
- Implementation of HT guidelines for HIE in France is inconsistent.
- Delayed HT initiation and non-adherence to indications are significant issues.
- Optimization of perinatal care is needed to improve HT practices for HIE newborns.
