Summary

A modified Molecular Adsorbent Recirculating System (MARS) with double adsorption units demonstrated enhanced efficacy in reducing bilirubin and bile acids in liver failure patients. This high-efficiency MARS (HE MARS) shows promise for improved liver support therapy.

Background:

  • Molecular Adsorbent Recirculating System (MARS) is a standard treatment for liver failure, generally well-tolerated.
  • Existing MARS systems face challenges in maximizing efficiency for severe liver dysfunction.

Purpose of the Study:

  • To enhance the efficiency and tolerability of the MARS liver support system.
  • To evaluate a modified MARS (HE MARS) with double adsorption units in parallel.

Main Methods:

  • Modification of the standard MARS albumin circuit by incorporating two adsorption units in parallel.
  • Treatment of four patients with severe liver failure (two with hyperbilirubinemia, two with high total bile acids) using the HE MARS system.

Main Results:

  • HE MARS achieved greater reductions in serum bilirubin compared to standard MARS in patients A and B (e.g., 52.3% vs. 27.6% in patient A).

Conclusions:

  • Further investigation with larger patient cohorts is warranted to confirm these findings.
