[A case of advanced gastric cancer successfully treated with multimodal therapy]

In November 2010, a 76-year-old man was diagnosed as having advanced gastric cancer with paraaortic lymph node (PALN) metastasis. Systemic chemotherapy was initiated using S-1/docetaxel( DTX) regimen. Regression of the PALN swelling was confirmed after 2 courses. Total gastrectomy with D2 lymph node dissection was performed as a reduction surgery, although intraoperative histopathological examination revealed PALN metastasis. Two months postoperatively, S-1 chemotherapy was initiated for the treatment of residual PALN metastasis. About 8 months postoperatively, abdominal computed tomography( CT) examination indicated complete regression of the PALN enlargement. This patient has been doing well for 30 months since the reduction surgery, with no sign of recurrence. In summary, we successfully treated a case of PALN metastasis with multimodal therapy including systemic chemotherapy and reduction surgery.
