Summary

Multidisciplinary treatment, including chemotherapy and radiation, achieved complete response in three gastric cancer patients with lymph node recurrence after surgery. This combination therapy shows promise for managing recurrent gastric cancer.

Background:

  • Gastric cancer recurrence, particularly lymph node metastasis, poses a significant challenge after curative gastrectomy.

Observation:

  • Three patients with advanced gastric cancer experienced lymph node recurrence (para-aortic, cervical, pancreatic head) following gastrectomy and D2 lymphadenectomy.
  • Treatments included concurrent chemoradiotherapy with S-1 and radiation, or sequential chemotherapy (irinotecan, cisplatin, 5-FU/Leucovorin) and radiation.

Findings:

  • Complete response (CR) was achieved in all three patients.
  • Concurrent chemoradiotherapy with S-1 plus radiation was effective for para-aortic and cervical lymph node recurrence.
  • Sequential chemotherapy and radiation therapy demonstrated efficacy for pancreatic head lymph node recurrence.

Implications:

  • Combined systemic chemotherapy and local radiation therapy represent a potentially effective treatment approach for lymph node recurrence in gastric cancer patients.
  • These findings support further investigation into multidisciplinary treatment protocols for recurrent gastric cancer.
