Background:

Orbital hydatid cysts are rare parasitic infections caused by Echinococcus granulosus.
Primary intraorbital hydatid cysts are exceptionally uncommon, with most cases secondary to other locations.

Observation:

An 80-year-old female presented with left eye proptosis and decreased vision.
Clinical examination revealed a non-tender, non-pulsatile proptosis.
Magnetic Resonance Imaging (MRI) identified a retro-bulbar cyst.

Findings:

Surgical excision via lateral orbitotomy was performed.
Histopathological examination confirmed the presence of a hydatid cyst.
The cyst was solitary and primary within the orbit.

Implications:

This case underscores the importance of considering parasitic infections in orbital diseases, even in elderly patients.
Early diagnosis and surgical intervention are crucial for preserving vision and orbital structures.
Highlights the need for awareness of rare presentations of echinococcosis in ophthalmology.
