Vasculitis simulating eczematous dermatitis due to C2 deficiency.

M Irestedt, T Månsson, A Svensson

    Acta Dermato-Venereologica
    |January 1, 1987

    Summary

Total C2 deficiency can lead to leukocytoclastic vasculitis and recurrent infections in some individuals. However, other patients with C2 deficiency may remain asymptomatic, highlighting varied clinical presentations.

    Area of Science:

    • Immunology
    • Genetics

    Background:

    • Complement system deficiencies, particularly C2 deficiency, are associated with increased susceptibility to infections and autoimmune diseases.
    • Homozygous C2 deficiency is a rare condition affecting the classical complement pathway.

    Observation:

    • A patient with total C2 deficiency presented with recurrent respiratory infections and a papulovesicular rash attributed to leukocytoclastic vasculitis.
    • The patient's brother, also with total C2 deficiency, was asymptomatic.

    Findings:

    • HLA genotyping confirmed the expected genotype for homozygous C2 deficiency in both affected and asymptomatic siblings.
    • Heterozygous carriers (mother and sister) exhibited moderately reduced C2 levels.

    Implications:

    • This case underscores the variable clinical expressivity of total C2 deficiency.
    • Understanding the genetic and clinical spectrum of complement deficiencies is crucial for accurate diagnosis and management.
