Summary

Alpha-fetoprotein (AFP)-producing renal cell carcinoma is a rare cancer. This study reports a case and reviews others, suggesting AFP production may be more common in mesoderm tumors than previously thought.

Background:

  • Alpha-fetoprotein (AFP) is a well-established tumor marker for hepatocellular carcinoma and yolk sac tumors.
  • Renal cell carcinoma (RCC) is a malignant tumor originating from the kidney, typically of mesodermal origin.

Observation:

  • A rare case of AFP-producing renal cell carcinoma with extensive bone and lung metastases, but no liver involvement, is presented.
  • Immunohistochemical staining confirmed the presence of AFP within the cytoplasm of renal tumor cells.
  • Three additional cases of AFP-producing RCC from Japanese literature were reviewed.

Findings:

  • The collective evidence from these four cases suggests that mesoderm-originating malignant tumors, including renal cell carcinomas, can produce AFP.
  • This indicates that AFP may be a more universal oncofetal antigen than commonly recognized.
