Background:

The sciatic nerve, the largest nerve in the human body, originates from the sacral plexus and innervates the hamstrings and all muscles of the lower limb below the knee.
Understanding sciatic nerve anatomy is crucial for surgical procedures and anesthesia in the lower limb.

Observation:

A bilateral anatomical variation in the formation of the sciatic nerve was observed in a single male human cadaver.
The sciatic nerve roots (lumbosacral trunk L4-L5, S1, S2, S3) remained separate until the lower gluteal region, deviating from the typical pelvic origin.

Findings:

The study identified a variant sciatic nerve formation where the single nerve trunk initiated in the lower gluteal region, not the pelvic region.
All contributing nerve roots maintained separate sheaths until this lower gluteal level.

Implications:

This anatomical variation necessitates increased awareness among surgeons and anesthetists regarding potential complexities in sciatic nerve blocks.
Failure of complete sciatic nerve blockade can occur despite multiple attempts if the nerve presents as separately sheathed bundles in the lower gluteal region.
Further investigation into the incidence of this variation in the general population is recommended to inform clinical practice.
