Background:

  • Lactococcus garvieae is an uncommon cause of infective endocarditis (IE) in humans.
  • Subdural haemorrhage (SDH) is a rare neurological complication of IE.

Observation:

  • An 81-year-old male with a prosthetic aortic valve presented with fatigue and headache.
  • CT revealed a subdural haematoma; blood cultures identified L. garvieae.
  • Transesophageal echocardiography showed vegetations on native and prosthetic valves.

Findings:

  • This is the first reported case of SDH as the sole neurological presentation of IE.
  • The patient received penicillin and tobramycin with an uneventful recovery.

Implications:

  • IE should be considered in patients with unexplained SDH, especially those with fever or prosthetic heart valves.
  • Early diagnosis and treatment are crucial for managing IE and its complications.
