Summary
Thoracic endometriosis, a rare condition, may be more common in West Africa. This study highlights its varied presentation and satisfactory treatment outcomes with a multidisciplinary approach.
Area of Science:
- Reproductive Medicine
- Thoracic Surgery
- Gynecologic Pathology
Background:
- Thoracic endometriosis syndrome (TES) is a rare condition involving intrathoracic endometrial tissue, with limited reports from African centers.
- Existing theories do not fully explain the diverse clinical presentations of TES.
- This study addresses the scarcity of data on TES in West Africa.
Purpose of the Study:
- To detail the demographics, pathological findings, and treatment outcomes for patients with intrathoracic endometriosis at a West African center.
- To contribute to the understanding of TES prevalence and characteristics in the region.
Main Methods:
- Retrospective review of 12 women diagnosed with intrathoracic endometriosis at Ghana's National Cardiothoracic Centre (2004-2012).
- Analysis of patient demographics, associated endometriosis locations, prior surgical history, and thoracic manifestations.
- Review of treatment modalities, including pleurodesis and hormonal therapy.
Main Results:
- Patients ranged from 24-39 years (mean 32 ± 5).
- Pelvic endometriosis was present in 66.7%, with ectopic endometrial tissue also found at the umbilicus (16.7%) and mons pubis (16.7%).
- 58.3% had prior uterine surgery. All 12 patients had right hemithorax involvement, with 50% experiencing pneumothorax, 41.7% hemothorax, and 8.3% hemopneumothorax.
Conclusions:
- Thoracic endometriosis might be underreported in West Africa.
- A multidisciplinary treatment strategy yields satisfactory outcomes for thoracic endometriosis.
- Further research is needed to elucidate the pathophysiology and optimize management of TES.

Thoracentesis(Thoracocentesis), commonly known as pleural tap, is a medical procedure where a 22 gauge needle is inserted into the pleural space, the area between the lung and chest wall. This procedure is commonly performed to diagnose or treat various respiratory disorders.
Description
Excess pleural fluid or air may accumulate in some respiratory disorders in the thoracic cavity. To treat pleural effusion, a physician conducts thoracentesis by carefully piercing the chest wall and entering...

Disorders of the Female Reproductive System
4.5K
The female reproductive system can be affected by several disorders, including Premenstrual Syndrome (PMS), Premenstrual Dysphoric Disorder (PMDD), endometriosis, and various forms of cancer. PMS and PMDD are cyclical conditions that cause physical and emotional distress, with symptoms that include edema, mood swings, and food cravings. PMDD is a more severe form of PMS characterized by increased symptom severity that peaks during the luteal phase and tends to improve or resolve shortly after...

Endoscopic Studies I: Bronchoscopy and Thoracoscopy
1.1K
Endoscopy is a non-surgical medical technique used to examine a person's internal organs and vessels. This lesson will focus on two types of endoscopic studies: bronchoscopy and thoracoscopy.
Bronchoscopy
Description
Bronchoscopy is a procedure that involves direct visualization of the larynx, trachea, and bronchi for diagnostic and therapeutic purposes. A flexible fiber optic or rigid bronchoscope is used to carry out the procedure. The fiber-optic bronchoscope is more frequently used due...

Cushing Syndrome I: Introduction
36
Cushing syndrome refers to the collection of clinical manifestations that arise when tissues are exposed to excessive amounts of cortisol or cortisol-like medications over an extended period. Cortisol, a glucocorticoid produced by the adrenal cortex, regulates metabolism, immune responses, and the body’s adaptation to stress. When its concentration remains chronically elevated, these physiological pathways become dysregulated, resulting in the characteristic features of the...

Intestinal Obstruction II: Pathophysiology
47
Intestinal obstruction triggers a series of physiological responses, starting with gas and fluid accumulation in the bowel segment proximal to the obstruction, leading to distension. This distended intestine compresses the diaphragm, hindering lung expansion and potentially leading to reduced respiratory effort, atelectasis, and pneumonia. To overcome the blockage, the gut intensifies contractions, causing colicky abdominal pain, nausea, and vomiting, which reduces fluid and food intake and...

Aneurysm II: Clinical Manifestations and Diagnostic Studies

Thoracic, aortic arch and abdominal aneurysms are significant vascular conditions that can present with various clinical manifestations and lead to serious complications. Understanding these manifestations and the appropriate diagnostic studies is essential for effective management and treatment.Thoracic Aortic AneurysmsThoracic aortic aneurysms often remain asymptomatic until they reach a size that impinges on adjacent structures. They typically cause deep, diffuse chest pain that radiates to...
