    Summary

    A rare intra-abdominal synovial sarcoma in the gastrocolic ligament presented as a mass and pain. Despite treatment, the patient experienced relapse, highlighting the aggressive nature of this sarcoma.

    Background:

    • Synovial sarcoma is a soft tissue sarcoma typically arising in extremities, rarely in the abdomen.

    Observation:

    • A 64-year-old woman presented with abdominal mass and pain.
    • CT revealed a large intra-abdominal mass compressing adjacent organs.
    • Biopsy confirmed biphasic synovial sarcoma of the gastrocolic ligament.

    Findings:

    • The patient received neoadjuvant chemotherapy, followed by surgical resection.
    • Post-operative adjuvant chemotherapy was administered.
    • Imaging after 18 months showed pulmonary and intra-abdominal disease recurrence.
