Background:

Deciduosis is a rare condition involving ectopic endometrial tissue.
It typically occurs in women of reproductive age, often during pregnancy or postpartum.
Peritoneal deciduosis is an uncommon manifestation, presenting as lesions on the peritoneum.

Observation:

A 32-year-old woman presented for a cesarean section due to breech presentation.
Intraoperative findings revealed over 30 yellowish, elastic nodules on the parietal peritoneum.
These lesions raised concerns for metastatic disease among the surgical team.

Findings:

Histological and immunohistochemical analyses confirmed the diagnosis of peritoneal deciduosis.
The diffuse distribution and appearance of the nodules mimicked metastatic lesions.
This case highlights an unusual presentation of peritoneal deciduosis.

Implications:

Accurate diagnosis of peritoneal deciduosis is crucial to avoid misinterpretation as malignancy.
Understanding this rare condition aids in surgical planning and patient management.
Further research may elucidate the pathogenesis and optimal treatment strategies for peritoneal deciduosis.
