Purpose of the Study:

  • To compare readmission rates between primary care practices undergoing transformation and those providing usual care.

Main Methods:

  • Four primary care clinics implemented a multicomponent intervention including enhanced care coordination and readmission monitoring.
  • Patient readmissions were analyzed over 12 months (May 2012-April 2013).
  • A comparison was made between patients from transformed practices (Group 1) and usual-care practices (Group 2).

  • A total of 961 patients were analyzed; 685 in Group 1 and 276 in Group 2.
