Disorders of the Male Reproductive System01:20

Disorders of the Male Reproductive System

5.2K
Men's health issues are increasingly recognized as significant, with several conditions posing common threats. Among these, testicular cancer is especially prevalent in younger men, particularly those aged 20 to 35 years. The disease often manifests as a painless mass in the testicles, sometimes accompanied by a sensation of heaviness or a dull ache.
Prostate disorders are another major concern. These conditions can impair urinary flow due to the prostate's location around the urethra....

Male Sexual Response: Erection & Ejaculation

Sexual stimulation can take various forms, such as physical touch and visual or auditory cues. When this happens, the parasympathetic reflex in the sacral portion of the spinal cord is activated. This reflex stimulates the release of nitric oxide (NO), which then dilates the arterioles in the penis, increasing blood flow to the erectile tissues - the corpora cavernosa and corpus spongiosum.
The blood filling the erectile tissues compresses the veins, which helps to prevent blood from leaving...

Infertility in Males01:23

Infertility in Males

665
Male infertility affects millions of couples worldwide, arising from various factors that impact different stages of the reproductive process. An endocrine imbalance resulting from conditions like hypogonadism, Klinefelter syndrome, or pituitary disorders can disrupt hormone levels and reduce sperm production. Testicular defects, such as tumors, cryptorchidism, atrophic testes, abnormal sperm morphology, and low sperm count or motility, may arise due to genetic factors, structural...

Urinary Tract Calculi III: Medical Management

The diagnosis of renal calculi involves several imaging techniques, including non-contrast CT scans and ultrasound. These methods help visualize kidney stones, assess their size and location, and detect possible obstructions. Additionally, Measuring urine pH is useful for diagnosing specific stone types, such as struvite (alkaline pH) and uric acid stones (acidic pH). Cystine stones are primarily linked to cystinuria, a genetic condition. A urinalysis helps detect blood in the urine (hematuria)...

Urodynamic Studies: Uroflowmetry

9.4K
Uroflowmetry is a non-invasive urodynamic test designed to measure various aspects of urination, including volume, flow rate, and the time to void. This test is crucial for diagnosing and assessing conditions such as bladder outlet obstruction, bladder dysfunction, incomplete bladder emptying, incontinence, and urinary tract blockages caused by benign prostatic hyperplasia (BPH) and urethral strictures.Pre-Test Instructions:Before a uroflowmetry test, patients are typically advised to drink...

Sperm Transport01:15

Sperm Transport

5.5K
The journey of sperm from its origin to the point of ejaculation begins within the seminiferous tubules of the testis. Here, Sertoli cells produce fluid that propels non-motile sperm through a series of conduits, starting with the straight tubules leading to the rete testis. This interconnected network of tubules acts as the initial pathway for sperm, guiding them into the efferent ductules and then into the epididymis for maturation.
The maturation phase occurs in the epididymis, where sperm...

Summary

Selective serotonin re-uptake inhibitors effectively treat premature ejaculation (PE). However, delayed ejaculation (DE) and anejaculation remain poorly understood, with limited treatment options and unproven therapies.

Area of Science:

  • Urology
  • Andrology
  • Sexual Medicine

Background:

  • Ejaculatory dysfunction, including premature ejaculation (PE), delayed ejaculation (DE), and anejaculation, significantly impacts quality of life.
  • Historically, PE treatment was limited to psychotherapy, but pharmacotherapy is now established.
  • DE and anejaculation are less common, less studied, and present significant challenges to sexual fulfillment and procreation.

Purpose of the Study:

  • To review the current understanding and treatment landscape of ejaculatory dysfunctions.
  • To highlight the efficacy of SSRIs for PE and the challenges in managing DE and anejaculation.

Main Methods:

  • Review of evidence-based studies on selective serotonin re-uptake inhibitors (SSRIs) for PE.
  • Analysis of literature concerning the epidemiology, aetiology, pathophysiology, and treatment of DE and anejaculation.

Main Results:

  • SSRIs are proven effective and safe as first-line treatments for lifelong and acquired PE.
  • Psychosocial aspects, epidemiology, aetiology, and pathophysiology of PE have gained increased attention.
  • DE and anejaculation lack robust research, with numerous psychotherapeutic approaches lacking large-scale validation.
  • Pharmacotherapy for DE and anejaculation has shown limited success, with no approved drugs and experimental options.

Conclusions:

  • SSRIs represent a cornerstone in PE management.
  • Significant unmet needs exist for effective and validated treatments for delayed ejaculation and anejaculation.
  • Further research is crucial to understand and treat DE and anejaculation effectively.
