    Background:

    • Posttraumatic stress disorder (PTSD) is a prevalent condition among veterans seeking U.S. Department of Veterans Affairs (VA) disability benefits.
    • Historically, women have faced greater challenges than men in obtaining PTSD disability benefits.

    Purpose of the Study:

    • To investigate the persistence of gender disparities in PTSD disability benefits over time.
    • To determine if clinical factors account for any observed gender differences.

    Main Methods:

    • A longitudinal, observational study of 2,998 U.S. veterans applying for VA PTSD disability benefits between 1994 and 1998.
    • Analysis of changes in PTSD service connection status over a ten-year period.
    • Gender-stratified, nationally representative sampling.

    Main Results:

    • Women had lower odds of gaining PTSD benefits (OR=0.70) and higher odds of losing them (OR=1.76) compared to men.
    • Adjusting for clinical factors explained the gender difference in gaining PTSD benefits.
    • Clinical factors and demographics eliminated the gender gap in losing PTSD benefits.

    Conclusions:

    • Gender-based disparities in receiving PTSD service connection benefits persisted over ten years.
    • Identified clinical factors and demographic characteristics explained these disparities.
    • Further investigation into gender disparities in the loss of PTSD disability benefits is recommended.
