Background:

Substance P (SP) is implicated in blood pressure regulation and the adrenergic system.
Animal studies suggest SP inhibits stress-induced noradrenaline increase.

Purpose of the Study:

To investigate Substance P levels and stress response in essential hypertension.
To explore the role of SP in hypertension pathogenesis and treatment.

Main Methods:

Measured SP-like immunoreactivity (SPLIR) in hypertensive and normotensive subjects.
Assessed SPLIR changes during a mental stress test.
Evaluated SPLIR response to antihypertensive drugs (prazosin, captopril).

Hypertensive patients exhibited significantly lower baseline SPLIR compared to normotensive controls.

Mental stress decreased SP in hypertensive patients but increased it in normotensive subjects.

Antihypertensive treatments (prazosin, captopril) increased plasma SP levels.

Conclusions:

Reduced plasma SP levels and blunted stress response may contribute to enhanced stress sensitivity in primary hypertension.
SP appears to be involved in the pathogenesis and therapeutic mechanisms of human hypertension.
