Purpose of the Study:

  • To investigate associations between comorbidity and medication use in patients with polysomnographically confirmed idiopathic REM sleep behavior disorder (iRBD).
  • To compare iRBD patients with a matched control group.

Main Methods:

  • A multicenter, clinic-based cohort study analyzed data from 318 iRBD patients and 318 matched controls.
  • Self-administered questionnaires assessed comorbidity and medication use.
  • Logistic regression analysis was used for comparisons.

Main Results:

  • iRBD patients showed higher rates of depression (OR 2.0) and antidepressant use (OR 2.2), particularly selective serotoninergic reuptake inhibitors (OR 3.6).
  • Increased prevalence of ischemic heart disease (OR 1.9) was observed in iRBD patients, independent of cardiovascular risk factors.
  • Higher use of inhaled glucocorticoids (OR 5.3) in iRBD patients was linked to increased smoking rates (OR 15.3) and pulmonary disease.

Conclusions:

  • The study confirms associations between comorbid depression and antidepressant use in iRBD.
  • An unexpected link between iRBD and ischemic heart disease, not explained by traditional risk factors, was identified.
  • These findings suggest a broader spectrum of comorbidities associated with iRBD.
