Summary

This case highlights a rare instance of retroperitoneal liposarcoma presenting with melena, a gastrointestinal bleed. The tumor infiltrated the ascending colon, requiring surgical resection of the affected colon and kidney.

Background:

Retroperitoneal liposarcomas are rare tumors often diagnosed late due to asymptomatic growth.
Diagnosis can be challenging, frequently requiring advanced imaging and biopsy.

Observation:

A 72-year-old male presented with melena, a rare initial symptom for retroperitoneal liposarcoma.
Colonoscopy revealed an ascending colon lesion, while CT identified a large retroperitoneal tumor.

Findings:

Biopsy and immunohistochemistry confirmed the ascending colon lesion as a well-differentiated or dedifferentiated liposarcoma, positive for CDK4 and MDM2.
Surgical resection revealed the retroperitoneal liposarcoma infiltrating the ascending colon and involving the right kidney.

Implications:

This case underscores the importance of thorough investigation for gastrointestinal bleeding, even with suspected retroperitoneal masses.
The presentation of melena and colonic mass highlights an unusual clinical course for retroperitoneal liposarcoma, emphasizing diagnostic variability.
