Summary

Patterned laser trabeculoplasty (PLT) caused less damage than argon laser trabeculoplasty (ALT) at sub-threshold settings in cats. However, both methods showed late scarring in the trabecular meshwork.

Background:

  • Argon laser trabeculoplasty (ALT) is a common glaucoma treatment.
  • Patterned laser trabeculoplasty (PLT) was developed to reduce thermal damage and improve coverage.
  • Comparative studies on the morphologic effects of PLT and ALT are limited.

Purpose of the Study:

  • To compare the morphologic changes in the feline trabecular meshwork after PLT and ALT.
  • To evaluate the impact of different energy levels of PLT on trabecular meshwork structure.

Main Methods:

  • Argon laser trabeculoplasty (ALT) was performed on the right eyes and PLT on the left eyes of 6 cats.
  • PLT was applied at either supra-threshold (400-450 mW) or sub-threshold (250-350 mW) energy levels.
  • Trabecular meshwork structural changes were assessed using light and scanning electron microscopy at 1, 4, and 9 weeks post-treatment.

Main Results:

  • Supra-threshold PLT induced coagulative damage and membranous obliteration, similar to ALT.
  • Sub-threshold PLT caused thinning and endothelial cell denudation but did not disrupt trabecular beams.
  • Late-stage partial membranous coverage was observed after sub-threshold PLT.

Conclusions:

  • Sub-threshold PLT demonstrated less thermal damage to the trabecular meshwork compared to ALT.
  • Neither sub-threshold PLT nor ALT effectively prevented late-stage scarring of the trabecular meshwork in cats.
