Remote and rapid pathological diagnosis in a resource challenged unit

Access to timely diagnostic services is limited in low-income regions like Malawi. Pediatric oncology services face challenges due to delayed or absent cytological diagnoses. Prior research has shown that telemedicine can bridge diagnostic gaps in underserved areas. However, no prior work had resolved how telepathology could be applied to pediatric oncology in such settings. The absence of rapid diagnostic tools hampers treatment decisions for cancer patients. Local diagnostic capacity is often constrained by limited expertise and infrastructure. Telemedicine offers a potential solution by connecting local teams with remote experts. This gap motivated the development of a telepathology program in Malawi.

The aim of this study was to evaluate the effectiveness of a telepathology program in improving diagnostic accuracy for pediatric oncology patients in Malawi. The specific problem addressed was the inability to make timely cytological diagnoses in most cases. The motivation was to provide a practical solution for low-resource settings. The program connected local teams with a remote diagnostic team. The goal was to assess the program’s impact on diagnostic accuracy and speed. The study focused on comparing remote and local diagnostic outcomes. It aimed to determine if telepathology could reduce diagnostic delays. The results could inform similar initiatives in other resource-limited regions.

The study evaluated a telemedicine program in Malawi’s pediatric oncology service. A total of 197 consecutive patients were included in the analysis. The majority of samples were fine needle aspirates, with others including bone marrow aspirates and peripheral blood films. Local and remote teams independently reviewed the samples. The remote team provided diagnoses and reports within 24 hours. Results were compared with local reports when available. The study tracked the number of diagnostic agreements and discrepancies. It also assessed the time taken to deliver remote reports.

A diagnosis was reached in 52% of the 197 samples analyzed. There were 46 discordant results between local and remote teams. Of these, 38 occurred when either team could not reach a diagnosis. Only 8 cases had differing diagnoses from the two teams. The remote team completed reports within 24 hours in all cases. Fine needle aspirates were the most common sample type. Bone marrow aspirates and peripheral blood films were also analyzed. The program demonstrated a positive impact on diagnostic turnaround times.

The telepathology program improved diagnostic accuracy and speed in Malawi’s pediatric oncology service. The remote team provided timely diagnoses in 52% of cases. Disagreements were mostly due to inability to diagnose rather than conflicting results. The program could be replicated in other low-resource settings. The findings suggest that telemedicine can support clinical decision-making. The rapid turnaround of remote reports is a key benefit. The authors propose that twinning with better-resourced centers can enhance diagnostic capacity. The study highlights the potential of telepathology in underserved regions.
