Background:

Acute myocardial infarction (MI) can lead to severe mechanical complications.
Ventricular septal defect (VSD) and pseudoaneurysm formation are rare but life-threatening sequelae of MI.

Observation:

A 77-year-old woman presented with acute MI complicated by VSD and a pseudoaneurysm.
Clinical presentation and echocardiographic findings were key to diagnosing this complex cardiac condition.

Findings:

The patient underwent successful surgical repair using a double patch technique.
This approach effectively addressed both the VSD and the pseudoaneurysm.

Implications:

Early recognition and prompt surgical management are vital for improving outcomes in post-MI VSD and pseudoaneurysm.
This case underscores the challenges and successful strategies in surgical repair of complex post-MI cardiac defects.
