Offspring of type 2 diabetes (T2D) patients show decreased insulin sensitivity (IS) with age. This decline in IS is an early sign of metabolic dysfunction in middle-aged adults, independent of obesity.

- OFD+ status was independently associated with increased fasting insulin levels and decreased IS.
Conclusions:
- Decreased insulin sensitivity may be an early detectable abnormality in middle-aged adults.
- The prevalence of reduced IS increases with age and is linked to having a parent with T2D, irrespective of obesity.
