Background:

  • Medical errors in pediatric and neonatal intensive care units (NICU) pose significant risks.

Purpose of the Study:

  • To determine the frequency, types, and adverse events associated with errors in a NICU and subintensive care units.
  • To analyze the correlation between hospitalization duration and error occurrence.

Main Methods:

  • Anonymous detection sheets were used to record errors over a two-month period (March-April 2010).
  • Data collected from 166 patients across one NICU and two subintensive care units.
  • Statistical analysis, including Spearman's correlation, was employed.

Main Results:

  • A total of 72 errors were documented, averaging 0.43 errors per patient.
  • 28% of patients experienced at least one error; 10% experienced more than one.
  • Medication administration errors accounted for 48% (NICU) and 53% (Subintensive CU) of all errors. A significant correlation (P<0.01) was found between hospitalization days and error rates.

Conclusions:

  • Vascular access errors caused severe harm in the NICU, while monitoring errors led to serious incidents in subintensive care.
  • Inattention and distraction were the primary causes of errors, followed by lack of experience and fatigue.
  • Study findings were disseminated to staff to promote awareness and improve patient safety.
