Is an annular tear a predictor for accelerated disc degeneration?

Annular tears (AT) in lumbar discs do not predict accelerated degeneration. A matched case-control study found similar degeneration progression rates in discs with and without ATs over four years.

- Discs with AT were matched 1:1 to control discs based on initial degeneration (Pfirrmann grade 3-4), age, BMI, and follow-up interval.

- Disc degeneration progression occurred in 25% of discs with an initial AT.
- A similar progression rate of 22% was observed in matched control discs without AT.
- No significant difference in the degree of degeneration progression was found between groups (p = 1.00).
Conclusions:
- Annular tears (AT) in lumbar discs with Pfirrmann grade >2 do not indicate a higher risk of accelerated degeneration.
- The presence of an annular tear alone is not a predictor of accelerated disc degeneration.
