Ovaries

3.5K
The ovaries are roughly the size of almonds and measure approximately 2 to 3 centimeters in length. These paired structures are situated within the pelvic region and are anchored by the mesovarium—a peritoneal extension that also connects them to the wider structure of the broad ligament. The support system extends to the suspensory ligament, housing blood and lymphatic vessels. In addition, the ovarian ligament tethers the ovaries to the uterus.

Area of Science:

  • Gynecologic Pathology
  • Endocrine Pathology
  • Oncology

Background:

  • Paragangliomas are rare neuroendocrine tumors typically arising in the adrenal medulla or sympathetic ganglia.
  • Ovarian paragangliomas are exceptionally rare, posing diagnostic challenges due to their unusual location.

Observation:

  • A 1.2-cm intraovarian mass was incidentally discovered in a 68-year-old female patient with endometrial carcinoma.
  • Histological examination revealed characteristic Zellballen architecture with polygonal clear cells exhibiting granular cytoplasm.

Findings:

  • Immunohistochemistry demonstrated diffuse expression of cytokeratin (CAM5.2), chromogranin, neuron-specific enolase, synaptophysin, and CD56.
  • S-100 protein was confined to sustentacular cells, aiding in differential diagnosis.
  • Distinguishing features from Sertoli cell tumors and ependymomas were highlighted, emphasizing the importance of Zellballen morphology and specific immunophenotypes.

Implications:

  • This case underscores the importance of considering paraganglioma in the differential diagnosis of rare ovarian neoplasms.
  • Pathologists must be aware of the potential for paraganglioma in unusual locations.
  • Accurate identification is crucial for appropriate patient management and prognosis.
