Background:

Subcutaneous phaeohyphomycosis is a fungal infection caused by dematiaceous fungi.
It primarily affects individuals with compromised immune systems.

Observation:

A kidney transplant recipient developed subcutaneous phaeohyphomycosis on their left hand.
The patient had been on a daily regimen of prednisone, tacrolimus, and azathioprine for three years.

Findings:

This case demonstrates a rare presentation of subcutaneous phaeohyphomycosis in an immunosuppressed transplant patient.
The infection occurred despite a 3-year history of consistent immunosuppressive medication.

Implications:

Highlights the risk of opportunistic fungal infections in solid organ transplant recipients.
Emphasizes the need for vigilance in diagnosing and managing fungal infections in immunosuppressed populations.
