Summary

Acute necrotizing pancreatitis (ANP) occurred in 0.5% of patients after endoscopic retrograde cholangiopancreatography (ERCP). Risk factors included obesity and difficult cannulation, with bacterial growth observed in severe cases.

Area of Science:

  • Gastroenterology
  • Endoscopy
  • Pancreatology

Background:

  • Acute necrotizing pancreatitis (ANP) is a severe complication.
  • Endoscopic retrograde cholangiopancreatography (ERCP) and endoscopic sphincterotomies (EST) are common procedures.

Purpose of the Study:

  • To analyze the incidence and characteristics of ANP following ERCP.
  • To identify potential risk factors and pathogenetic mechanisms of post-ERCP ANP.

Main Methods:

  • Retrospective analysis of patients undergoing ERCP.
  • Evaluation of ANP incidence, patient demographics, indications for ERCP, procedural details, and outcomes.
  • Microbiological analysis of bile, blood, and ascitic fluid.

Main Results:

  • ANP incidence was 0.5% for ERCP and 0.5% for EST.
  • Affected patients were obese, middle-aged or older women.
  • Symptoms began within 6 hours post-ERCP; pancreatitis was severe and necrotizing.
  • Bacterial growth (E. coli, Str. faecalis, Klebsiella pneumoniae) was common.

Conclusions:

  • Post-ERCP ANP is a rare but severe complication.
  • Obesity and challenging cannulation techniques may increase risk.
  • Bacterial infection appears to play a role in the pathogenesis of severe ANP.
