A new image analysis technique precisely quantifies ocular bulbar injection. This objective method differentiates conjunctival and scleral vessel injection, improving ocular surface assessment.

Area of Science:

  • Ophthalmology
  • Medical Imaging
  • Biomedical Engineering

Background:

  • Ocular bulbar injection is a key indicator of ocular surface inflammation and disease.
  • Current methods for assessing injection are often subjective and lack precision.
  • Objective quantification of regional ocular injection is needed for accurate diagnosis and monitoring.

Purpose of the Study:

  • To develop and validate an objective, image-based method for precise evaluation of regional ocular bulbar injection.
  • To differentiate between conjunctival and superficial scleral vessel injection using quantitative image analysis.

Main Methods:

  • Six healthy volunteers were photographed in four orientations with and without stimulating eye drops.
  • ImageJ software was used to draw line segments and analyze graph peaks representing vessel grey-level.

  • Quantitative parameters including peak area, peak height/width (PH/PW), and peak numbers were analyzed.
  • Main Results:

    • The developed method demonstrated a smaller coefficient of variation, particularly for PH/PW, across all orientations.
    • PH/PW ratios were consistently higher in conjunctival vessels (>0.87) than in superficial scleral vessels (<1.00).
    • Conjunctival vessels showed a significant increase in PH/PW, while superficial scleral vessels exhibited minimal changes.

    Conclusions:

    • A novel, objective image-based method for assessing ocular bulbar injection has been successfully developed.
    • This technique allows for precise quantification of regional ocular injection.
    • The method effectively distinguishes between conjunctival and superficial scleral vessel involvement in ocular injection.
