    Background:

    • Obesity and weight gain are established risk factors for psoriasis.
    • Psoriasis patients exhibit a higher prevalence of obesity than the general population.
    • Limited research exists on the specific role of dietary interventions in managing psoriasis.

    Purpose of the Study:

    • To evaluate the effectiveness of a combined dietary and physical exercise intervention for weight loss.
    • To assess the impact of this intervention on improving psoriasis severity in overweight or obese individuals.

    Main Methods:

    • 303 overweight/obese patients with moderate-to-severe chronic plaque psoriasis were included.
    • Participants received either a 20-week structured diet and exercise plan or informational counseling.
    • The primary outcome measured was the reduction in Psoriasis Area and Severity Index (PASI) score at 20 weeks.

    Main Results:

    • The dietary intervention group showed a median PASI reduction of 48%, compared to 25.5% in the information-only group (P=0.02).
    • A PASI score reduction of ≥50% was achieved by 49.7% of patients in the intervention group versus 34.2% in the control group (P=0.006).
    • Weight loss targets (≥5% reduction) were met by 29.8% of intervention participants, significantly higher than the 14.5% in the control group (P=0.001).

    Conclusions:

    • A 20-week dietary intervention combined with physical exercise effectively reduced psoriasis severity.
    • This approach is beneficial for overweight or obese patients with active psoriasis receiving systemic treatment.
    • Weight management through diet and exercise can be a key strategy in controlling psoriatic disease activity.
