Background:

  • Diffusion-weighted imaging (DWI) is crucial for assessing neonatal brain development and pathological conditions like hypoxic-ischemic encephalopathy (HIE).
  • Quantitative assessment of diffusion values is challenging, especially with rapid myelination changes during development.
  • Diagnosing neonatal HIE is complicated by global ischemic injury during critical developmental periods.

Purpose of the Study:

  • To compare quantitative histogram analysis of apparent diffusion coefficient (ADC) maps with qualitative MRI interpretation.
  • To correlate diffusion value changes with gestational age in normal neonates.
  • To evaluate the sensitivity of histogram analysis as a quantitative measure for neonatal HIE.

Main Methods:

  • Reviewed neonatal brain MRI studies over 4 years, identifying normal (23) and HIE (12) cases.
  • Performed histogram analysis of ADC values from a single basal ganglia slice.
  • Correlated peak (s-sDav) and lowest (s-sDlowest) histogram values with gestational age.

Main Results:

  • Normative s-sDav values showed a significant linear decline with increasing gestational age (r²=0.477, P<0.01).
  • Six of 12 HIE cases exhibited significantly lower s-sDav and s-sDlowest ADC values compared to normative data.

Conclusions:

  • Single-slice histogram display of ADC values offers a rapid and feasible quantitative analysis method for neonatal brain diffusion.
  • Normative ADC values correlate with gestational age, declining linearly through the perinatal period.
  • While capable of identifying some HIE cases, the overall sensitivity of this histogram method for detecting HIE is low.
