Rous Sarcoma virus or RSV was discovered by F. Peyton Rous in the year 1911 as a filterable transmissible agent that could cause tumors in chickens. He won a Nobel Prize for this discovery in 1966. Background:

  • Low-grade extraskeletal osteosarcoma is exceptionally rare, with limited literature, particularly involving the retroperitoneum.
  • Management strategies for this neoplasm are not well-established due to its rarity and potential for dedifferentiation.

Observation:

  • A case of the largest reported retroperitoneal low-grade extraskeletal osteosarcoma (18.0 cm × 18.3 cm × 17.8 cm) in a 38-year-old male is presented.
  • The patient underwent surgical tumor excision.

Findings:

  • Post-operative treatment included six cycles of cisplatin and doxorubicin chemotherapy, administered every three weeks with granulocyte colony-stimulating factor support.
  • The patient achieved a disease-free status at the 3-year follow-up mark.

Implications:

  • This case highlights a successful multimodal treatment approach for a rare retroperitoneal tumor.
  • Further case reports and literature reviews are crucial for refining management protocols for low-grade extraskeletal osteosarcoma.
