Summary

LigaSure Small Jaw (LSJ) and Harmonic Focus (HF) devices demonstrated equivalent safety and effectiveness in open total thyroidectomy for papillary thyroid carcinoma (PTC). Both surgical devices resulted in similar operative times, complication rates, and hospital stays.

Background:

  • LigaSure Small Jaw (LSJ) and Harmonic Focus (HF) are energy devices used in thyroid surgery.
  • Their comparative efficacy in open total thyroidectomy for papillary thyroid carcinoma (PTC) requires evaluation.

Purpose of the Study:

  • To compare the safety and effectiveness of LSJ and HF devices in open total thyroidectomy for PTC.

Main Methods:

  • A prospective, randomized study included 126 patients undergoing open total thyroidectomy for PTC.
  • Patients were divided into two groups: 64 receiving LSJ and 62 receiving HF.
  • Key outcomes analyzed included operative time, drain output, parathyroid status, complications, laboratory data, hospital stay, and analgesia requirements.

Main Results:

  • No significant differences were observed in operation time, parathyroid status, postoperative complications (including hypocalcemia), or hospital stay between the LSJ and HF groups.
  • The LSJ group showed significantly lower drain output (106.8 vs 123.6 mL, P = .01).
  • Ionized calcium levels were higher on postoperative days 1, 2, and 10 in the LSJ group (P = .04, P = .04, P = .01).

Conclusions:

  • Both LSJ and HF devices are safe and effective for open total thyroidectomy in PTC patients.
  • Surgical outcomes and operative morbidity were equivalent between the two devices, with no increased complications noted.
