Summary

Intraoperative cell salvage systems significantly reduce the need for allogeneic blood transfusions in scoliosis patients undergoing spinal fusion surgery. Routine use is recommended, especially for those with low hemoglobin or long procedures.

Area of Science:

  • Orthopedic Surgery
  • Transfusion Medicine
  • Anesthesiology

Background:

  • Allogeneic blood transfusions carry inherent risks.
  • Minimizing allogeneic transfusions is a key goal in spine deformity surgery.
  • Intraoperative cell salvage offers a potential solution for blood loss management.

Purpose of the Study:

  • To evaluate the efficacy of intraoperative cell salvage in reducing allogeneic transfusions.
  • To assess the safety of cell saver systems in scoliosis patients undergoing spinal fusion.
  • To identify factors predicting transfusion needs in this patient population.

Main Methods:

  • 110 scoliosis patients undergoing posterior spinal fusion were randomized into cell saver and control groups.
  • Data collected included perioperative hemoglobin, blood loss, and transfusion rates.
  • Statistical analyses (Chi-square, t-tests, regression) were used to compare groups and identify predictors.

Main Results:

  • The cell saver group had a significantly lower allogeneic transfusion rate (14.5% vs. 32.7%, p=0.025).
  • Mean intraoperative red blood cell transfusion was lower in the cell saver group (0.21 U/pt vs. 0.58 U/pt, p=0.032).
  • Predictors of transfusion included fused segments, preoperative hemoglobin, and cell saver use.

Conclusions:

  • Cell saver use significantly decreases allogeneic blood requirements in spine deformity surgery.
  • The system is particularly beneficial for patients with low preoperative hemoglobin or prolonged surgery.
  • Routine cell saver utilization is confirmed as beneficial for scoliosis patients undergoing PSF.
