Intravenous regional anesthesia or the Bier block technique is used to anesthetize a specific limb or extremity. It uses exsanguinated or blood-drained vessels to transport local anesthetics or LAs to the peripheral nerve trunks. Lidocaine without vasoconstrictors like epinephrine is most commonly used for this technique. Other drugs used are prilocaine, ropivacaine, and chloroprocaine. Bupivacaine is not recommended for this technique due to its high cardiac toxicity.

Summary

Free toe soft tissue transfer under local anesthesia offers a viable option for fingertip reconstruction in patients unsuitable for general anesthesia. This microsurgical technique, especially with a two-team approach, provides effective tissue coverage with reduced operative time and anesthetic use.

Background:

  • Free toe soft tissue transfer is a reconstructive option for fingertip defects.
  • Local infiltration anesthesia is explored for patients unsuitable for general anesthesia.

Observation:

  • Eleven traumatic fingertip defects were reconstructed using free toe soft tissue transfer.
  • Donor sites included the lateral great toe and medial second toe.
  • Follow-up ranged from 8 months to 2.5 years.

Findings:

  • Flap sizes ranged from 1.0 × 2.0 to 2.0 × 3.5 cm.
  • Mean operative times were 4 hours 29 minutes (single team) and 3 hours 21 minutes (two-team approach).
  • All flaps survived with one case of partial necrosis; minimal local anesthetic was used.

Implications:

  • Fingertip reconstruction via free toe transfer under local anesthesia is feasible and effective.
  • A two-team microsurgical approach can further optimize operative time and reduce anesthetic requirements.
  • This method presents a realistic alternative for patients with contraindications to general anesthesia.
