Status 2 heart failure patients have a poor prognosis, similar to those on inotropes or mechanical circulatory support (MCS). High plasma B-type natriuretic peptide (BNP) may indicate suitability for ventricular assist device (VAD) therapy in this group.

- Ten Status 2 patients died, primarily from sudden death (hemodynamic compromise or ventricular fibrillation).

- Prognosis for Status 2 patients is poor, often due to sudden death without MCS.
- Status 2 patients with elevated plasma BNP levels are potential candidates for continuous-flow VAD therapy.
