Focal encephalopathy is a rare side effect of cisplatin chemotherapy. Symptoms like vision loss and seizures can occur, but often resolve with supportive care, allowing continued cisplatin treatment.

Background:

  • Cisplatin is a widely used chemotherapy agent.
  • Focal encephalopathy is a rare but serious adverse effect of cisplatin therapy.

Observation:

  • Four patients developed focal encephalopathy after cisplatin administration.
  • Symptoms included cortical blindness, aphasia, hemianopia, and seizures.
  • Episodes occurred during febrile neutropenia without CNS infection.

Findings:

  • Neurological symptoms appeared after at least two cisplatin courses (≥300 mg/m2).
  • Hypomagnesemia was present in three of four patients.
  • All neurological deficits resolved completely with supportive care.

Implications:

  • Focal encephalopathy associated with cisplatin is often reversible.
  • Complete resolution may allow for continued cisplatin therapy.
  • Monitoring magnesium levels could be important in patients receiving cisplatin.
