Laparoscopic common bile duct exploration in elderly patients: is there still a difference?

Background:

• Common bile duct stones (CBDS) pose a treatment challenge in elderly patients due to perceived high surgical risks.
• Laparoscopic common bile duct exploration (LCBDE) is an alternative to open surgery, but its safety and efficacy in the elderly are not well-documented.

Purpose of the Study:

• To evaluate the safety and efficacy of laparoscopic common bile duct exploration (LCBDE) in elderly patients (age 70+) compared to younger patients.
• To assess postoperative complications, reoperation rates, and mortality associated with LCBDE in different age groups.

Main Methods:

• A retrospective analysis of 94 patients who underwent LCBDE between February 2004 and January 2012.
• Patients were divided into two groups: age 70 or older (Group A, n=38) and under 70 (Group B, n=56).
• Data analyzed included comorbidities, ASA scores, conversion rates, bile duct clearance, complications, reoperations, and mortality.

• Elderly patients had more preoperative risk factors but similar stone extraction success rates (89.5% vs. 96.4%).

• Age should not be a barrier to offering LCBDE for choledocholithiasis.
